2006 APHA Distinguished Public Health Legislator of the Year Award (Deadline June 19, 2006)

Established in 1997, the awards, which honor legislators who have taken courageous actions to support public health, will be presented during APHA's Annual Meeting in Boston, Mass., on November 4-8, 2006.

When considering federal legislators for nomination, we recommend that you review APHA's annual congressional vote record, available on the APHA Members Only Web site at:
http://www.apha.org/private/congressrecord06.pdf . All legislators, regardless of the number of nominations, will be considered equally.

Please use this form to submit a Legislator of the Year Award nomination, which should include the legislator's full name, state/city of residence, a biography and a detailed explanation of why the legislator is being nominated to receive the award .

The winners of the award will be chosen by the Executive Board.
